Healing Arts Social Work Services is actively seeking a dedicated Licensed Master Social Worker (LMSW) to join our compassionate team in Pueblo, Colorado. Work Environment

This position is primarily remote, allowing for flexibility in your day-to-day schedule while ensuring that seniors and their families receive timely support. While your primary hours will be Monday to Friday from 8:00 AM to 4:30 PM, there may be instances where evening or weekend hours are necessary based on the individual needs of clients.

Key Responsibilities
  • Conduct comprehensive assessments to evaluate the psychosocial well-being of seniors, factoring in emotional, mental, and social conditions.
  • Collaborate with clients and their families to craft personalized care plans that align with their preferences and goals.
  • Provide compassionate counseling and emotional support, assisting clients in managing the challenges and transitions associated with hospice care.
  • Act as an advocate for clients' rights, ensuring they have access to necessary community resources and support services.
  • Work closely with an interdisciplinary team, including nurses and caregivers, to streamline care processes and enhance the overall client experience.

Patient Population / Client Focus

The ideal candidate will work primarily with seniors in a hospice setting, addressing the holistic needs of clients nearing the end of their life. This includes providing emotional support, practical resources, and facilitating a peaceful transition for clients and their families.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
  • Valid and current state licensure as a Licensed Master Social Worker (LMSW).
  • Minimum of two years of experience in a social work role, specifically in geriatric care or a community-based setting is essential.
  • Strong knowledge of social work principles, aging-related issues, and dynamics within hospice settings.
  • Exceptional assessment, counseling, and problem-solving skills.
  • Ability to demonstrate empathy, compassion, and build rapport with seniors and their families.
  • Excellent communication and interpersonal skills, essential for effective collaboration with colleagues and families.
