Nurse Practitioner (Full-Time) - Post-Acute / SNF Focus
Greater Chicago Area

We are seeking an experienced Nurse Practitioner (NP) to join us as a foundational clinical hire in the Greater Chicago market. This is a unique, early-stage opportunity to help build and shape a consultative clinical model focused on hydration health in skilled nursing facilities, working closely with both Corporate and Clinical Leadership.
This role is ideal for a clinically strong, entrepreneurial NP who thrives in a high-autonomy environment and is excited to contribute to something being built from the ground up.

About the Role
As a core clinical partner, you will work alongside leadership to design, implement, and deliver a new model of care within the SNF and post-acute setting.
  • Serve as a consultative provider focused on renal and hydrational health focused care
  • Manage a daily patient volume of approximately 13-15 patients
  • Partner with Corporate and Clinical Leadership to develop protocols, workflows, and standards of care
  • Support program development in an early-stage environment where structure is still evolving
  • Build strong relationships with facility staff, administrators, and interdisciplinary teams
  • Contribute to long-term strategy, with potential to grow into a clinical leadership role
Responsibilities
  • Deliver high-quality, patient-centered care in skilled nursing and post-acute facilities
  • Assess, diagnose, and manage patients with a focus on hydration status and related conditions
  • Collaborate with internal teams to refine clinical pathways and treatment protocols
  • Provide clinical insight to support operational and programmatic decisions
  • Ensure compliance with all regulatory and clinical standards
  • Travel locally within the Greater Chicago market as needed
Qualifications
  • Active Nurse Practitioner license in Illinois
  • Minimum 2-3 years of NP experience
  • Strong experience in skilled nursing or post-acute care required
  • Proven clinical judgment and ability to work independently
  • Excellent interpersonal and relationship-building skills
  • Comfortable in a startup or early-stage environment with evolving structure
  • Passion for building, improving, and scaling clinical programs
