Hospice RN Case Manager - Key Responsibilities

  • Provide skilled nursing care and symptom management to terminally ill patients.

  • Develop and manage individualized care plans in coordination with the hospice team.

  • Educate and support patients and families through end-of-life care.

  • Administer medications and treatments for comfort and quality of life.

  • Coordinate care with physicians, social workers, aides, and other team members.

  • Respond to urgent needs and provide crisis support when necessary.

  • Maintain accurate and timely documentation in the EMR.

  • Ensure compliance with hospice regulations and standards.

Qualifications:

  • Active Washington State Registered Nurse (RN) license.

  • Prior Home Health and/or Hospice experience is strongly preferred.

  • Current BLS certification from the American Heart Association.

  • Valid driver's license and insured, reliable vehicle.
