???? Role Overview

This is a field-based hospice RN role where you will provide compassionate, end-of-life care to patients in their homes and community settings.

???? Schedule:

  • Weekend Program

  • Saturday & Sunday (8:00 AM - 8:00 PM)

  • No case management responsibilities


????‍⚕️ Patient Care Focus
  • End-of-life hospice patients (life expectancy ≤ 6 months)

  • Common diagnoses:

    • ~40% Cancer

    • ~60% Chronic/other conditions

Visit Types:

  • Daily end-of-life visits

  • PRN symptom management

  • Occasional routine/stable visits


???? Workload
  • 5-6 patient visits per day (8-hour equivalent)

  • Field-based care across:

    • Patient homes

    • Assisted living facilities

    • Nursing homes

    • Group homes


???? Experience & Requirements
  • Minimum 2 years RN experience (Required)
