Kidspath, a service of AuthoraCare Collective, is currently seeking a PRN RN Home Care Case Managerfocused in ourBurlingtonservicearea. This can include care in other areas including Chapel Hill and Durham. This position is as needed, but able to provide consistent, flexible hours. Room for expansion of hours for a good fit. Willing to train to the skillset for someone with experience in pediatrics.CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Job Summary:

Manage RN case load for all/multiple services of Kids Path program (may include hospice, home health, perinatal services), in collaboration with other members of the interdisciplinary Team (IDT). Assess and identify patient/caregiver complications, establish patient/caregiver care goals and identify and implement interventions. Work with physician to establish and implement medical plan of care for the patient.

Establish and maintain therapeutic relationship with patient and caregiver. Work closely with other members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations. Availability after-hours occasionally required at the discretion of the Kids Path leadership team. Program support responsibilities required in combination with field responsibilities.Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
