Home Health Registered Nurse (RN) responsibilities include:

  • providing services in accordance with the plan of care
  • making the initial evaluation visit and regularly reevaluating the patient's nursing needs, as well as ensuring the plan of care needs are met and making any necessary revisions
  • monitoring and evaluating patients' progress and updating care plans accordingly
  • educating patients and their families on managing the conditions and promoting wellness
  • informing physician and other personnel of changes in the patient's condition and needs, as well as collaborating with interdisciplinary healthcare team members to coordinate the best possible care
  • participating in weekend clinical on-call rotation.
  • participating in in-service programs, supervising and teaching other nursing personnel as necessary

Requirements for the Home Healthcare Registered Nurse position:

  • valid Registered Nurse (RN) license in the state of Illinois
  • minimum of one year of nursing experience, preferably in home health or a related setting
  • proficient in using electronic medical record systems, such as Axxess
  • previous experience in geriatrics and home health
  • ability to work independently and prioritize tasks effectively
  • ability to provide care with empathy, respect, and understanding
  • strong communication and interpersonal skills
