PRN Float Registered Nurse (pool 2 - NOC shift)

The Float B PRN registered nurse is responsible for rotating duties between the community, the Meng Residence, and the Triage department, depending on staffing needs. This includes completing skilled nursing assessments within the community or at the Meng Residence, as well as providing safe and effective telephone advice, care instructions, and directions to the appropriate level of care. The Float B nurse works collaboratively with the primary care teams, interdisciplinary team, and schedule coordinators to achieve high-quality patient standards.
PRN Requirements: Pool 2 - Must work a minimum of 6 shifts in a 3-month period. At least one holiday per year, and weekends as determined by the leader/designee based on departmental need.
Hours: Overnight shifts, including 6:00 pm-6:00 am, 8:00 pm-8:00 am, or 11:00 pm-7:00 am.
Knowledge, Skills, and Abilities:
  • Computer proficiency (MS Office - Word, Excel, and Outlook)
  • Highly motivated and organized with a strong attention to detail and analytical skills to help solve problems.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment.
  • Ability to keep up to date with policies, practices, and regulations.
  • Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures.
  • Comply with all applicable local, state, and federal laws and regulations as well as policies and procedures of Unity.
  • Satisfy mandatory training requirements regarding local, state, and federal regulations and/or Unity's policies and procedures.
  • Perform other job-related duties as assigned.
  • Ability to adjust to frequently changing demands.

Key Job Duties:
  1. Provide skilled nursing assessment and appropriate intervention
  2. Update the plan of care as necessary and review care plans with patient and family
  3. Respond to urgent patient/family needs in collaboration with triage nursing staff
  4. Initiate crisis care as needed
  5. Report appropriate findings and significant changes to the primary team, attending physician and primary caregiver
  6. Provide education to patient/family members on disease progression, symptom management, medication and other associated patient care needs
  7. Administer and titrate medication according to physician/nurse practitioner orders
  8. Mange individual patient medication and Residence contingency medications
  9. Record all received orders to the Electronic Medical Record and initiate follow up as needed
  10. Prioritize daily work and make adjustments taking into account the needs of patients, Unity and other staff
  11. Communicate with interdisciplinary team members
  12. Delegate nursing duties to LPN and supervise care provided by CNA
  13. Complete new patient admissions as needed
  14. Telephone triage and follow-up to patient care related contacts
  15. Record and maintain complete and accurate records of patient and physician communications
  16. Fax/call new and refill prescriptions according to physician orders
  17. Coordinate volunteer deliveries

Minimum Qualifications & Certification/Licensure:
  1. Associate degree in Nursing required, bachelor's degree preferred.
  2. RN license in the state of practice required (RN must practice under the law and regulations governing nursing in the state of practice).
