Job description

Private Duty Nurse (RN/LPN) – 1:1 Home Care

Location: Westland, MI and surrounding area
Setting: In-home, one-on-one care

Job Summary

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients' homes. You'll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.

High-Acuity Pediatric Openings: West Detroit & WestlandWest Detroit: Infant & Toddler Care
  • 6-Month-Old Baby Boy: A brave little guy with interstitial lung disease. Care includes Trach, Vent, and NG-tube. He has his own room in a very clean, professional home. Non-smokers preferred.

    • Shifts: 7:00 AM – 7:00 PM (All days available, Mon–Sun).

  • 1-Year-Old Baby Boy: A very happy, smiley baby who loves being read to! He is Trach, Vent, and G-tube dependent. Parents are very organized and helpful.

    • Shifts: 7:00 PM – 7:00 AM (Saturdays).

  • 2-Year-Old Girl: Requires a skilled RN with pediatric trach/vent experience. Very helpful parents and a friendly cat (kept upstairs).

    • Shifts: 9:00 AM – 7:00 PM (Mondays, E/O Saturday, & PRN).

  • 4-Year-Old Boy: An active child who loves being outside and having fun! Trach, Vent, and G-tube management. Home has two small hypoallergenic dogs.

    • Shifts: 6:00 AM – 6:00 PM (Fridays).

Westland: Sweet Weekend Opportunity
  • 1-Year-Old Girl: A very sweet little girl with Trisomy 21. She is Trach and G-tube dependent and on a ventilator 24/7. She lives in a super cozy, clean home with very nice parents.

    • Shifts: 7:00 AM – 7:00 PM (Every Other Saturday)


Responsibilities
  • Provide skilled nursing care (trach/vent care, medication administration, catheter care)
  • Monitor and document patient condition and vital signs
  • Assist with daily living activities (ADLs)
  • Communicate with families and care teams
  • Maintain safe, high-quality, patient-centered care
Schedule & Pay
  • Full-time, part-time, and per diem shifts available
  • Day, night, and weekend shifts
  • Weekly pay + optional daily pay
