Coverage Territory: Effingham and surrounding areas

Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

  • Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
  • Building and adjusting personalized care plans in collaboration with physicians and care teams
  • Administering medications and treatments per physician orders
  • Keeping clinical documentation accurate, timely, and meaningful
