Job description

RN Triage/Weekend Coordinator Homecare 16 hours inc. every other weekend

Department:Home Care Clinical Admin

As a Triage RN/Weekend Coordinator you will be responsible will be to insure that Home Care patients receive all available services on weekends and holidays in accordance with Middlesex Hospital objectives, standards and policies. Processes new referrals from all referrals sources.

Minimum Qualifications:

  • A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
  • A non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.
  • All candidates must be computer literate and able to work is multiple systems.
  • Must be able to multi-task and may be required to visit patients.
  • Homecare experience
  • Certification in specialty area preferred.
