It is mandatory to use facility timeclock. Clinicians must pay for badge replacement if lost.

Type of facility: Long-Term Care/Skilled Nursing Facility. Total beds: 300 divided into 6 units. Ratios vary per unit.

Majority of floor nurses are LPNs. Days will have 3 Nurse Managers assigned to 2 units Monday through Friday. Weekend staffing includes one Nurse Manager for oversight.

Each unit has 2 additional nurses (one Charge nurse and one Medication/Treatment nurse). Nursing staff consists of both RNs and LPNs. Evenings will have 1 RN Supervisor and additional floor nurses.

Nights will have 1 RN Supervisor when staffing allows and 1 floor nurse per unit. Using 5 of 6 units means that 1 floor nurse will split between the two largest units. The RN Supervisor is responsible for running the building during evenings, nights, and weekends.

The RN Supervisor serves as a resource for others throughout the shift. The role includes monitoring changes in resident conditions and issues. Outside services such as lab and portable x-ray need to be scheduled.

Contacting physicians or nurse practitioners for resident needs is expected. Only RNs will perform central line care, chest tube management, IV starts, and fall assessments. Accepts residents 18 years or older with Long-Term Care, skilled, rehab, and dementia diagnoses.

Hospice care is accepted in-house with dedicated rooms. House physicians and nurse practitioners are available by phone after hours. Interdisciplinary team includes physical therapy, occupational therapy, speech-language pathology, case managers, social workers, and dieticians.

Respiratory therapy is not on-site 24/7. Pharmacy is available on-site during normal business hours. Medication is administered from pill bottles.

RN Supervisors have emergency drug closet access when not in pharmacy hours. Shifts are 8.25 hours including a 30-minute unpaid lunch break. Weekend work is required every other weekend, with the option to work more.

Holiday work is required based on the schedule. No call or standby requirements exist. No floating requirements for this position.

Scrub colors should be clean and professional, except for camo. Unit orientation lasts for 3 days.
