Position Summary:

NHC Scott is now hiring a full time patient care coordinator. Position Summary: Responsible for fall program, working with insurance r/t patient transportation/appointments, 1/5wks weekend only call and supporting the day to day nursing team. Working with insurance r/t patient transportation, oversee hospital transfers/readmits, assist IC nurse and support the day to day of the nursing team. Monday - Friday 7:00 am-3:30 pm hours are flexible/negotiable (On Call) 1/5 weekends Friday 5pm-Monday 8a

Licensure: Graduate from a nursing program accredited by the State. Maintains a current unencumbered licensed to practice in the state where employed.
