Nurse Practitioner (NP) – Primary Care / Family Medicine

Location: West Hartford, CT

Job Type: Full-Time or Part-Time

Our West Hartford primary care practice is expanding, and we are looking for a compassionate, motivated, and patient-centered Nurse Practitioner (NP) to join our team. Position Overview

The NP will deliver comprehensive outpatient primary care, including preventive services, chronic disease management, acute visits, and patient education. You will work closely with physicians and clinical staff while maintaining the independence to manage your own panel.

Key Responsibilities

  • Perform patient assessments, physical exams, and wellness evaluations
  • Diagnose and manage both acute and chronic medical conditions
  • Prescribe medications and therapeutic treatments per state guidelines
  • Order, interpret, and act on laboratory and diagnostic results
  • Provide preventive care, screenings, and annual physicals
  • Educate patients on disease management, medications, and lifestyle changes
  • Maintain accurate and timely documentation in the EHR
  • Collaborate with physicians, specialists, and support staff to ensure continuity of care

Qualifications

  • Master’s or Doctorate from an accredited Nurse Practitioner program
  • Active Connecticut NP license
  • Board certification (AANP or ANCC)
  • DEA license (or eligibility to obtain)
  • Current BLS certification
  • At least 1 year of experience in primary care, family medicine, or internal medicine preferred
  • Strong communication skills and commitment to patient-centered care

Schedule

  • Monday to Friday
  • No weekends or limited weekends depending on candidate preference
