Evaluates the overall effectiveness of care provided by other direct care givers. Coordinates the patient care in conjunction with other departments. 4C- serves the medical cardiac population by managing chronic and acute onset diseases while balancing the need for telemetry and cardiac management.  The types of patients we serve are those with multisystem management needs such as heart failure, respiratory distress, unmanageable dysrhythmias, pneumonia, sepsis, COPD, diabetes and gastro-intestinal conditions.  We also coordinate management of patients needing cath lab, angio, dialysis, outpatient infusion/chemo, and anti-coagulation therapies.
