Registered Nurse On Call Hospice
Overview:

Schedule is every Saturday/Sunday and one float day a week (clinician's choice). 8am-8pm

What You'll Do as a Hospice RN โ€“ After Hours / On-Call:


โ€ข Report directly to the Administrator, Executive Director, or Patient Care Manager Senior
โ€ข Provide direct patient care and support after normal business hours
โ€ข Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift
โ€ข Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs
โ€ข Evaluate patient and family needs to guide clinical decisions and implement the plan of care
โ€ข Support and collaborate with an interdisciplinary group including LPNs/LVNs, CNAs, and other support staff
โ€ข Accurately document clinical visits and communications
โ€ข Participate in quality assessment and performance improvement programs
โ€ข Work independently while ensuring continuity of care and coordination with the daytime care team

Compensation may vary within the salary range provided based on several factors including but not limited to a candidateโ€™s location, experience, education, skills, licensure, certifications and department equity.  Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Salary Range โ€“ $78,595 - 98,270

About You:

Experience - What you'll bring:


โ€ข Current RN license in the state of requested employment (or eligibility to obtain)
โ€ข 1+ years of nursing experience
โ€ข A hospice heart: compassionate, calm, and dependable in urgent care situations
โ€ข Excellent clinical judgment and the ability to provide support in emotionally intense settings
โ€ข Comfortable providing care in patientsโ€™ homes, facilities, or wherever they reside
โ€ข Valid driverโ€™s license, automobile insurance, and reliable transportation
โ€ข Current CPR certification

Preferred Experience (Not Required):


โ€ข 1+ years of hospice, home health, oncology, or palliative care experience
โ€ข Familiarity with symptom management, triage, and after-hours home-based care
โ€ข Prior experience in on-call or overnight nursing roles
โ€ข Strong documentation and electronic medical records (EMR) skills
