Job Summary

We are seeking a Registered Nurse to join our team at Angel Care Hospice and Palliative. In this role, you will primarily be creating and managing a plan of care for individual patients. Our hospice team is seeking a Registered Nurse (RN) who operates with great attention to detail and a big heart. Our patients reside in their own residence or residential facility throughout Los Angeles County. Our RNs take pride in creating personalized care plans for patients and are able to advocate and communicate effectively in the patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as excellent communication and time management skills. Angel Care Hospice & Palliative is recognized in the community for providing outstanding end-of-life care for our patients and being a dependable resource for families and caregivers. Our Hospice RNs work as part of an interdisciplinary team including Physicians, Social Workers, LVNs, CHHAs, Chaplains and other support staff. If your passion is to provide warm and compassionate care for terminally ill patients at their most critical time in life, this is the perfect opportunity to demonstrate those skills and make a difference.

Schedule is Monday-Friday 10-7pm with alternating weekends.

Responsibilities

Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans.

Participate in IDT meetings to discuss patient progress and assess level of needs.

Communicate effectively with hospice team regarding patient needs.

Administer medications and monitor patients for side effects or reactions

Document any changes in patient conditions and report on changes timely and accurately.

Promote high quality and effective interventions and outcomes.

Maintain accurate and timely records of patient care and discuss observations with the supervisor.

Adhere to professional standards as outlined by protocols, rules and regulations.

Qualifications

Currently licensed Registered Nurse (RN) in state of California

Previous experience as a Registered Nurse is preferred

Excellent interpersonal and client care skills

Valid driver’s license

First aid/CPR certification required

Compassionate and a team player

Excellent organizational and time management skills
