What You'll Do
  • Provide skilled hospice nursing care in patients' homes

  • Assess patient needs and manage pain and symptoms with compassion and clinical expertise

  • Develop and implement individualized plans of care in collaboration with the interdisciplinary team

  • Educate and support patients, families, and caregivers throughout the hospice journey

  • Maintain accurate, timely clinical documentation

  • Coordinate care with physicians, therapists, social workers, chaplains, and other members of the hospice team

  • Participate in a shared on-call rotation to ensure continuous, high-quality patient care

What You Bring
  • Current Arizona Registered Nurse (RN) license

  • Minimum one year of nursing experience (hospice, home health, or palliative care preferred)

  • Associate's or Bachelor's degree in Nursing

  • Current CPR certification

  • Valid Arizona driver's license, reliable transportation, and current auto insurance

  • Excellent clinical assessment and communication skills

  • A compassionate, patient-centered approach to care

  • Ability to work independently while collaborating effectively with an interdisciplinary team
