What You'll Do

  • Respond to after-hours calls and urgent patient needs throughout the service area.
  • Complete emergency hospice visits to assess changes in condition and provide expert symptom management.
  • Deliver pain management, medication administration, wound care, and other comfort-focused nursing interventions.
  • Provide compassionate telephone support and education to patients, families, and caregivers.
  • Coordinate closely with physicians and the interdisciplinary hospice team to ensure continuity of care.
  • Complete accurate and timely EMR documentation.
