 Job Summary for Registered Nurse Case Manager (RNCM)

  • Plan, organize and direct hospice care services, from initial evaluation and assessment, pain and medication management, and care plan development.
  • Ensure high quality patient experience and positive health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on patient needs. 
  • Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team and collaborate with the patient's primary care physician to ensure continuity of care.
  • Provide coaching and education to patients and their caregivers based on clinical diagnosis, empowering them to self-manage their condition, prevent complications, and improve quality of life as a Registered Nurse Case Manager (RNCM)
  • Able to cover San Mateo and San Francisco on weekends.

Minimum Qualifications

  • Current CA Registered Nurse (RN) license 
  • Current CPR card for Healthcare Providers
  • 1 year of Registered Nursing experience required
  • 1 year of Registered Nurse (RN) in hospice preferred
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
  • Self-directed and able to work with minimal supervision.
