Duties

  • Conduct comprehensive patient assessments, including vital signs, symptom management, and psychosocial evaluations.
  • Administer medications and treatments such as IV infusions, pain management protocols, and wound care, ensuring adherence to hospice standards.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with patient wishes and clinical needs.
  • Provide education and emotional support to patients and their families, guiding them through complex medical decisions and end-of-life processes.
  • Maintain accurate medical documentation within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
