Physical Therapist - Home Health

Location: Sunnyvale, CA

Start Date: 09-08-2026

Contract Type: Travel Contract

Shift Type: Days, 8:00 AM - 5:30 PM (flexible based on patient availability); one weekend day per month

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities
  • Evaluate and treat patients using medically prescribed physical therapy programs to restore function and prevent disability
  • Make initial admission and evaluation visits, and notify the Clinical Team Manager of OASIS scores within established timelines
  • Supervise Physical Therapy Assistants and support the care team
  • Document observations, clinical findings, problems, skilled interventions, goals, and discharge plans
  • Work collaboratively with patients, families, and the interdisciplinary team to help each person reach their maximum performance
Qualifications
  • One year of Home Health experience
  • Active Physical Therapist license in California (must be currently active)
  • BLS/BCLS certification
  • A passion for providing compassionate, patient-centered care in the home health setting
Knowledge & Skills
  • Knowledge of home health and hospice care principles
  • Skilled in physical therapy evaluation, treatment, and care planning
  • Experience with OASIS documentation and compliance
  • Ability to supervise and delegate to Physical Therapy Assistants
  • Strong communication and documentation skills
