Outpatient Physical Therapist
Lebanon, OR | Contract | 13 Weeks | Days (8:00 AM – 4:30 PM, No Weekends)

Job Summary

The Outpatient Physical Therapist provides comprehensive evaluation and evidence-based treatment for patients with conditions that affect movement, strength, balance, and functional independence in an outpatient setting. This role manages a varied caseload across orthopedic, sports, neurological, balance, and general rehabilitation populations, focusing on restoring function, reducing pain, and promoting long-term wellness.​

Key Responsibilities
  • Perform comprehensive physical therapy evaluations, including history, systems review, movement analysis, and standardized outcome measures, to determine functional impairments and limitations.​

  • Develop and implement individualized, goal-oriented treatment plans using therapeutic exercise, neuromuscular re-education, manual therapy, balance training, and functional activities.​

  • Provide outpatient physical therapy interventions to improve mobility, strength, range of motion, gait, balance, activity tolerance, and overall functional independence.​

  • Treat a variety of conditions including orthopedic and sports injuries, post-surgical cases, neurological disorders, chronic pain, and balance/vestibular dysfunction as appropriate to the clinic’s population.​

  • Document evaluations, daily treatment notes, progress reports, re-assessments, and discharge summaries accurately and timely in the EMR (EPIC) in accordance with regulatory and payer requirements.​

  • Educate patients and caregivers on diagnosis, treatment plans, body mechanics, injury prevention, and individualized home exercise programs to support carryover and self-management.​

  • Collaborate with referring providers, rehabilitation colleagues, and interdisciplinary teams to coordinate care, update plans of care, and optimize patient outcomes.​

  • Adapt treatment approaches based on patient response, clinical findings, and outcome measures; modify goals and plan of care as needed.​

  • Travel to multiple points of care (e.g., outpatient clinics or affiliated locations) as required while maintaining productivity and documentation expectations.​

  • Adhere to professional, ethical, safety, and infection control standards, as well as state practice act and organizational policies.​

Required Qualifications & Skills
  • Active, unencumbered Oregon Physical Therapist license.​

  • BLS (Healthcare Provider level) certification.​

  • Minimum 1 year of Physical Therapy experience; prior outpatient rehabilitation experience strongly preferred.​

  • Strong clinical reasoning and examination skills in orthopedic, neurological, and general outpatient rehabilitation.​

  • Excellent written and verbal communication skills for patient education, interdisciplinary collaboration, and documentation.​

  • Effective time-management skills with the ability to manage a full outpatient caseload and meet productivity and documentation expectations.​

  • Ability to work independently, prioritize competing demands, and adapt to multiple care settings and scheduling needs.​

  • Proficiency or familiarity with EPIC EMR or similar electronic documentation systems.​

Schedule & Contract Details
  • Shift: Days, 8:00 AM – 4:30 PM.​

  • Weekends: None; holiday coverage required per departmental schedule.​

  • Job Type: 13-week contract, 1 position available, with potential for extension based on need and performance.​
