Founded in 1941, Dignity Health - Dominican Hospital is a 188-bed, acute care, nonprofit hospital located in Santa Cruz, California. Serving over 80,000 patients annually, the hospital offers a full complement of services including heart care, orthopedics, and a Family Birthing Center. Additionally, Dominican Hospital has been recognized as an LGBTQ+ Healthcare Equality High Performer by the Human Rights Campaign Foundation. As our Nurse Practitioner, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.

Every day you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers. To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.

The Mobile Wellness Clinic midlevel provider is a licensed nurse practitioner (N.P.) or licensed physician assistant (P.A.) responsible for the provision of primary care and urgent/episodic care to the clinic's patients. The P.A. or N.P. provides a full scope of ambulatory care services largely geared to underserved populations. This care consists of comprehensive health screenings, establishing medical diagnosis for acute and chronic health problems, evaluation of normal and abnormal findings, initiation of treatments and referral services. These functions will be performed in accordance with standardized Procedures/Process Oriented Protocols approved by the Dominican Hospital Credential/Inter Disciplinary Practice Committee.

The midlevel provider will educate the patient and/or family about preventive care, medical issues, diagnosis and use of prescribed medical treatments and medications. The provider will maintain legible, accurate and confidential medical records in compliance with state regulatory standards and clinic policies and procedures. The midlevel provider is part of a multidisciplinary practice that offers a blend of individual and population-based care. Reports to CHIS Director, Clinic Medical Director and Program Coordinator.


Required
  • Bachelor’s Degree 
  • Current California Registered Nurse licensure, Nurse Practitioner licensure, and Nurse Practitioner Furnishing Number OR current Physician Assistant licensure
  • Certification of Family Nurse Practitioner by California Board of Registered Nurses
  • Current Basic Life Support-Provider Level certification
  • Current and valid CA Drivers License
  • Computer literate
