Position Overview

Key Responsibilities

• Conducting thorough psychosocial assessments to thoroughly understand clients' mental health challenges, strengths, and available resources.
• Delivering individual, couple, family, and group therapy sessions rooted in evidence-based practices, including cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.
• Designing and implementing individualized treatment plans that reflect the unique goals and needs of each client.
• Collaborating with psychiatrists, psychologists, and a multidisciplinary treatment team to ensure comprehensive and coordinated mental health care.
• Providing crisis intervention, conducting risk assessments, and developing safety plans in urgent situations.
• Facilitating group therapy sessions, psychoeducational classes, and support groups to enhance community support and interpersonal connections.
• Maintaining accurate and timely documentation in compliance with relevant standards and regulations.

Patient Population / Client Focus

Our clientele ranges across all age groups and backgrounds, including individuals experiencing challenges such as anxiety, depression, PTSD, and other emotional disturbances. Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) must be held.

• Demonstrated knowledge of evidence-based therapeutic modalities and techniques is a must.
• Excellent interpersonal, communication, and assessment skills are vital for effective client engagement and support.

Preferred Experience

• Experience working in a hospice or palliative care setting is highly desirable.
• Familiarity with a variety of therapeutic modalities that address different aspects of mental health.
• Ability to navigate electronic health record systems for documentation and case management.
