The Cognitive Care Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to provide compassionate care to seniors and their families in a remote capacity. This role operates within a remote setting, allowing for flexibility in service delivery to clients. Our remote team works collaboratively to ensure consistent communication and care integration, benefiting from digital tools for assessments and documentation. While the position primarily complies with a Monday to Friday schedule, from 8:00 AM to 4:30 PM, occasional evening or weekend hours may be necessary to accommodate client needs. On-call support may also be required to handle urgent situations effectively.

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting comprehensive assessments of seniors' psychosocial needs while considering emotional and social factors.
• Developing, implementing, and monitoring individualized care plans in coordination with clients and their families.
• Providing therapeutic counseling and emotional support during challenging life transitions, enhancing clients' coping abilities.
• Advocating for patients accessing vital community resources and services, ensuring equitable care.
• Collaborating with a multidisciplinary team of nurses, caregivers, and healthcare professionals to optimize care delivery.
• Regularly visiting clients to assess their living conditions, monitor well-being, and manage safety concerns.
• Facilitating conversations to resolve family conflicts regarding client care, promoting teamwork and understanding.
• Educating clients and families about community engagement opportunities, support groups, and available resources.
• Maintaining thorough documentation of client interactions, assessments, and progress in accordance with best practices and regulatory policies.

This role focuses primarily on seniors, many of whom may be experiencing complex health challenges, isolation, or transitions in living arrangements. The social worker will be instrumental in addressing emotional, psychological, and social needs while promoting health and independence. Understanding the unique backgrounds and experiences of elderly clients will be key in building trust and delivering effective services.

• Master's degree in Social Work from an accredited university.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of two years of clinical social work experience, with a preference for experience in geriatric or community health settings.
• In-depth knowledge of social work principles and aging issues, along with a network of available community resources.
• Strong assessment, counseling, and problem-solving skills to aid clients in achieving positive outcomes.
• Exceptional interpersonal skills, fostering rapport with clients, families, and interdisciplinary teams.
• Competency in electronic health record (EHR) systems and effective use of digital communication tools.

Experience in providing social work services to varied populations, particularly seniors and individuals with cognitive impairments, will be beneficial. Familiarity with geriatrics is crucial, as is an understanding of the challenges faced by this demographic in accessing care and resources.

We offer a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
