Flexible Schedule: AFTER 4PM & WEEKENDS PREFERRED


Join CG Behavioral Health & SUD Services as a Diagnostic Assessor and be part of a team dedicated to providing crucial clinical assessment and support to teens in need. This position involves evaluating client and family functioning, conducting mental health diagnostic assessments, and recommending treatment options. You'll work in various settings, including community-based locations and client homes, to ensure comprehensive care delivery.


RESPONSIBILTIES:

  • Conduct clinical assessments and outreach services for clients and their families.
  • Perform mental health diagnostic assessments according to DSM-5 criteria.
  • Maintain up-to-date knowledge of diagnoses and recommend treatment options.
  • Complete administrative tasks, including record-keeping and report generation.
  • Collaborate with other agencies and departments to ensure comprehensive care.
  • Maintain client confidentiality and uphold agency standards.
  • Provide availability for diagnostic evaluation appointments.
  • Inputs clinical documentation in the EHR system in the appropriate format and submits all documentation according to Caregiver Grove Behavioral Healths Policies & Procedures Manual.
  • Responsible for weekly reporting to the Clinical Services Director.
  • Performs all job duties in a manner that is friendly, considerate, and aligned with the organizations Mission, Values, Patient Care Philosophy, and Standards of Ethical Conduct.
  • All other duties as assigned.


QUALIFICATIONS:

  • Bachelor's degree in Counseling, Social Work, or a related Behavioral/Mental Health.
  • Requires license to practice as a Social Worker/ Counselor in Ohio (LSW, LISW, LPC, LPCC).
  • Ability to use a computer; proficiency in Word and Electronic Health Record (EHR).
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
  • Must have valid Drivers license and acceptable driving record.
