Hours: 40 hours per week; Monday, Wednesday, Thursday and Friday, 8:00 am to 4:30 pm and Tuesday, 10:00 am to 6:30 pm
This is a hybrid position that combines elements of both Case Management and Rehabilitation Specialist duties.
As a Case Manager / Rehabilitation Specialist, you will:
  • Conduct case management, linkages, and referrals to needed supports in the community. Act as a liaison between the program and community partners to facilitate referrals and coordination of care for WTW clients.
  • Assist clients and families with SSI applications; obtaining childcare; connecting with primary care, arranging medical and dental appointments; filling out applications and forms; and completing applications and forms for services.
  • Assist clients in arranging transportation and teach how to use public transportation including learning routes.
  • Provide individual and group rehabilitation and family activities.
  • Assist clients in meeting client service plan goals, prioritizing areas of need and steps needed to reach goals as well as identified areas of strength.
  • Assist clients with educational and vocational goals, conduct employment assessments and assist clients in linkage to schools, volunteering and/or employment opportunities.
  • Work with various businesses and agencies in the community to build relationships to support client job development and self-sufficiency.
  • Provide client transportation as needed.

Requirements
Education: Associate's degree or higher is required
Additional Qualifications and Skills
  • Minimum two years of experience working in the mental health field
  • Bilingual Spanish preferred, but not required
  • Intermediate computer skills in Microsoft Office, including Outlook, Word, and Excel.
  • A valid California drivers license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position
  • Able to pass criminal/background clearance
