Registered Nurse Case Manager – Direct Hire

Location: Dingmans Ferry, PA Schedule: Monday–Friday, 8:00 AM–6:00 PM Additional Requirements: 1 weekend per month + 2 holidays per year Travel: Local travel required across multiple counties

Overview

Join a compassionate, mission‑driven hospice team as a Registered Nurse Case Manager. In this role, you will provide essential care, guidance, and support to patients and families navigating end‑of‑life needs. You'll work closely with interdisciplinary teams, lead care coordination, and ensure patients receive the highest standard of hospice care.

Key Responsibilities
  • Manage and oversee patient care within hospice settings
  • Coordinate care plans with physicians, social workers, and interdisciplinary teams
  • Provide leadership, mentorship, and support to nursing staff
  • Conduct patient assessments and develop individualized care plans
  • Ensure compliance with hospice standards, policies, and regulatory requirements
  • Maintain accurate documentation and timely reporting
  • Travel locally to visit patients across assigned counties
Required Qualifications
  • Education: High School Diploma or GED
  • Experience:
    • 1+ year Hospice experience
    • 1+ year Management or Leadership experience
    • 1+ year experience in ANY of the following:
      • Home Health Care
      • Epic Home Health
      • Home Health
  • Licensure:
    • Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
  • Certifications (ALL required):
    • American Heart Association Basic Life Support (AHA BLS)
    • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Schedule & Shift Details
  • Monday – Friday, 8:00 AM – 6:00 PM
  • On‑call/Additional:
    • 1 weekend per month
    • 2 holidays per year
Travel Requirements
  • Local travel required across multiple counties
  • Must be comfortable working in field‑based hospice environments
