Job description


New River Valley Community Services Medical Services is seeking a Case Manager! The Case Manager provides needs assessments, links resources, and monitors services for individuals with substance use and/or co-occurring disorders with the MAT (Medication Assisted Treatment) Team. Theis role coordinates services and resources for clients as needed to maintain in their communities; in addition to monitoring services as they are delivered to ensure consumers' needs are met. The Case Manager also ensures documentation process requirements are met accordingly along with assistance with transportation and community integration as needed.
Pay Range: $21.83-$25.10- Depending on Experience
Education Requirements:
  • Bachelor's Degree in Education, Social Work, Psychology or Human Services related field is required with at least one year experience in a setting working with individuals with disabilities, mental health needs, substance use or Developmental Disabilities is preferred.
  • QMHP is required through the Virginia Board of Counseling.
  • CSAC is beneficial.
  • Previous Case Management experience is preferred.

Additional compensation will be considered for any new employees that speak multiple languages.
