As a PT Registered Nurse (RN) Case Manager, you’ll oversee home health aide services, ensuring they meet high-quality standards. PT Registered Nurse Case Manager Responsibilities: * Reports directly to Director of Operations * Work closely with Clinical Coordinator to ensure our clients and employees receive BrightStar standard level of care * Will work with both skilled and nonskilled cases/assignments * Can provide field training/orient new employees to cases * Can/will open skilled cases/assignments and maintain * Field visits as needed * Communication with Clinical Coordinator with any observed positive or negative in the field * Responsible for assessments/evaluations/reassessments/training/supervisor visits * Will assist with LRV assessments * Other (TBD) PT Registered Nurse Case Manager Requirements: * Current unrestricted RN license in state for which care is being provided * Minimum one (1) year of case management experience preferred. * Minimum one (1) year of clinical experience within a home health agency, primary care clinic or health facility, preferred. * With Reliable transportation with auto insurance * Clean background and no criminal record * Understands regulatory compliance with state and federal regulations * Adhere to HIPAA and maintain client confidentiality.
