Job Description

DEPARTMENT: Alexander Cohen Hospice House (ACHH)

REPORTS TO: ACHH Administrator

SUMMARY: As a member of the multidisciplinary team, the Hospice Aide (CHHA) works under the direction of the RN Administrator and the supervision of the registered nurse in the provision of personal care, in order to maximize the comfort and quality of life of patients and families. Responsible for providing supportive care in accordance with the plan of care and CHHS and ACHH policies and procedures.

RESPONSIBILITIES:

  • The Hospice Aide assists with home-like environmental services such as homemaking services to preserve a safe, sanitary patient personal space and to enhance quality of life in accordance with the Hospice Aide assignment and the hospice plan of care.
  • The Hospice Aide assists the patient with personal care and supportive services in accordance with the CHHA assignment and the hospice plan of care.
  • The Hospice Aide will communicate with other hospice team members as needed to report patient changes and needs and requested changes in the care assignment related to patient changes.
  • Documentation will be completed and placed in the patient chart.
  • Communicate with patient and family throughout the shift to provide care and encourage involvement whenever possible.
  • Attend regularly scheduled care conferences (IDT) to review cases, problem-solve and cover unique issues, share professional support and exchange feedback aimed toward enhancing professional growth.
  • Attend death events and provide appropriate support at time of death to patient/family, as requested.
  • Participation in CHHS orientation of students.
  • Participate in inservices, workshops, and clinical meetings to promote professional growth and understanding of hospice care.
  • Follows all CHHS and ACHH policies and procedures including, but not limited to, safety, infection control, privacy and confidentiality, and HIPAA.
  • Maintain a professional appearance of self and working area; exhibit appropriate behavior with guests and clients, i.e., courteous, thoughtful and professional at all times.

Qualifications
  • Experience in providing homemaking and personal care services in the home setting highly desirable.
  • Possesses and maintains current CPR certification.
  • Valid California Home Health Aide Certificate must be maintained at all times.

WORK ENVIRONMENT: Positions in this class are situated in a home-like environment where there is little or no exposure to variations in the weather or other similar elements.

WORK SCHEDULE: Required to work a regularly scheduled 36-hour workweek (12 hours per day), for full-time employees and with week-end rotation. Part-time employees will work 12-24 hours per week. Per Diem employees work less than 20 hours per week. Overtime as needed to maintain patient care needs.
