Job description

Join the Sentara Community Care Team at an exciting time of growth and impact! We are seeking a compassionate and flexible Family Nurse Practitioner who is passionate about serving the Hampton Roads and Northeast North Carolina communities. Our team provides essential primary care services through a network of community clinics and mobile health units, caring for uninsured and Medicaid populations. Position Highlights:

  • Locations: Hampton Roads VA and Northeast North Carolina
  • Part-Time: 20 hours/ 0.5 FTE
  • Working from Mobile Care RV providing direct primary care services to children and adults in underserved communities  
  • Treat family medicine, pediatrics, and adults with complex issues
  • Shared call schedule, with RN first call
  • Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically.
  • Review quality and outcome metrics monthly with the healthcare team
  • Collaborate with local civic and faith-based leaders on community education programs
  • Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
  • 1+ year APP experience required
