Comprehensive Assessor

The Area Agency on Aging is seeking a dedicated Comprehensive Assessor to support individuals and caregivers in accessing long-term care services and community-based resources. This role is responsible for conducting in-depth, multi-dimensional assessments to evaluate physical, functional, social, and environmental needs, determine eligibility for Medicaid and waiver programs, and develop person-centered care plans.

Key Responsibilities
  • Determine eligibility for Medicaid and waiver programs based on assessment findings.
  • Develop and implement person-centered care plans tailored to individual needs.
  • Maintain accurate and timely documentation in compliance with state and agency guidelines.
  • Collaborate with individuals, caregivers, healthcare providers, and community resources to coordinate care.
  • Ensure adherence to PASSPORT standards and procedures throughout the assessment process.
Qualifications
  • Current licensure as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Licensed Social Worker (LSW).
  • Minimum of one year of experience in home care for elderly and/or disabled adults, home health care, medical social work, geriatrics/gerontology, or approved substitutes as per ODA guidelines.
  • Strong clinical judgment and excellent assessment skills.
  • Knowledge of Medicaid, waiver programs, and PASSPORT standards and procedures.
  • Effective communication skills, both verbal and written.
  • Ability to manage documentation and ensure compliance with state and agency requirements.
