Puget Sound Home Health & Hospice, providing exceptional care for over 30 years, is expanding!

We're seeking a dedicated Home Health Clinical Manager for our Pierce County division. This role requires a Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license and experience in a Medicare-certified home health agency (OASIS trained).

Responsibilities:

  • Ensure compliance with home health/hospice laws and regulations
  • Review referrals and manage assignments
  • Oversee patient charts and clinical courses
  • Assist with staff training, hiring, and mentoring
  • Participate in QAPI, OASIS review, and coding workflow
  • Some direct patient care and on-call duties required

Requirements:

  • Minimum 2 years of healthcare management experience
  • At least 1 year of home health/hospice experience with OASIS training
  • BSN degree preferred
  • Current BLS certification
  • Washington state license
  • Experience with Home Care Home Base preferred
  • Knowledge of Medicare COPs, PDGM, VBP
  • Licensed and insured driver
