Job description

Aliya Health Group is a nationwide network of addiction treatment centers known for diverse, evidence-based programs for substance use disorders and co-occurring mental health disorders. Position: Case Manager | Royal Life Center Lacey | Full-Time |

Thursday - Monday 9:30am - 6:00pm

We are looking for a dynamic Case Manager to support clinical, non-clinical, and medical staff in the provision of technical and auxiliary clinical services to ensure comprehensive and effective daily programming and functionality. Specific responsibilities include conducting initial assessments, group counseling, case management services, and aftercare planning.

Job Duties: Case Manager

  • Assists in the development, implementation, and revision of Case Manager treatment plans; assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goals
  • Meet with patients weekly for Case Management
  • Completes all case management documentation in accordance with regulatory standards
  • Collect, monitor, and document urinalysis samples and breathalyzer screenings for all CLIA-approved waived testing
  • Facilitates referrals to physicians, dentists, and other healthcare professionals as indicated by the assessed needs of each individual patient
  • Provides transportation in accordance with transportation policy and procedures
  • Conducts Vocational Assessments for clients and ensures follow through
  • Provides group counseling services in accordance with the established clinical program and completes all documentation accordingly.
  • Collaborates with individual counselors to ensure coordination of appropriate aftercare planning for each client
  • Facilitates the completion of discharge surveys for clients upon coordination of aftercare planning
  • Gathers completed admission, mid-treatment, and discharge surveys with the purpose to aggregate data and compiling a monthly report.
  • Facilitates groups as scheduled by Clinical Supervisor/Clinical Director
  • Maintains an active case management caseload providing interventions as needed and within the area of expertise and limits of credential
  • Provides ASAM-specific reviews as needed and reports findings to the Utilization Review Specialist in a timely and efficient manner
  • Other duties as assigned

Requirements:

  • HS Diploma or GED
  • A valid driver’s license and a clean driving record
  • Fingerprint Clearance Card
  • Strong interpersonal skills
  • Case Management experience
  • Knowledge of all Microsoft office suites including, but not limited to: Word, Excel, PowerPoint, Outlook etc.
  • Versed in the use of Electronic Medical Records
  • Clinical knowledge pertaining to substance use disorders

Preferred Qualifications:

  • Experience in detox, residential, or inpatient medical/behavioral health preferred.
  • Experience performing intake, assessment, treatment planning, and coordination of multidisciplinary services.
  • Familiarity with care coordination involving medical providers, therapists, MAT programs, probation/parole, and community resources.
  • Knowledge of withdrawal management, substance use disorders, cooccurring disorders, and relapseprevention support.
  • EHR proficiency 
  • Ability to handle highvolume communication (email/fax/scanning)
  • Microsoft Office (Word/Excel) proficiency
  • High emotional resilience and calm under crisis
  • Strong organization/time management
  • Ability to work in fastpaced detox environment
  • Reliability and followthrough (critical for legal/medical coordination)
  • Willing to run group with up to 15 clients
