Job description

Job Title: Client Care Coordinator
Supervisor: Client Care Manager
Status: Part-Time (Monday-Friday 25 hours/week), non-exempt
Qualifications
  • Desire to work with disenfranchised populations while supporting TEEG's mission
  • Bachelor's degree in human services or related field or 5 years of similar work experience
  • Proficiency in Microsoft Office software
  • Strong written, oral, organizational, and conflict-resolution skills
  • Ability to work independently and collaboratively with flexible hours and duties
  • General knowledge of local and regional services and supports
  • Clean record with State Police Bureau of Identification, Department of Children and Families and Department of Motor Vehicles
  • Possesses a valid driver's license, reliable transportation, and proof of registration and motor vehicle insurance
  • Respect for persons without regard to age, race, color, national origin, ancestry, religion, sex, pregnancy, transgender status, gender identity or expression, sexual orientation, disability, genetic information, marital/civil union status, veteran status, uniformed servicemember status or socioeconomic status

Physical/Mental Requirements & Working Conditions
  • Frequent stationary work in an office or community setting to include keyboarding and/or handwriting
  • Daily working hours may vary to meet client and agency needs, including occasional early morning, evening, and weekend requirements
  • Occasional local travel in a personal or agency vehicle
  • Frequent one-on-one interactions with clients who may exhibit an emotionally heightened or withdrawn state and/or with behavioral diagnoses that may require de-escalation, crisis prevention or intervention
  • Frequent in person, virtual, and/or telephonic oral communication and listening
  • Ability to lift and/or move objects up to 30 pounds

Essential Functions
  • Maintain strict confidentiality of client and agency data and records
  • Maintain strong knowledge of all TEEG services and make inter-agency referrals as necessary
  • Maintain strong working knowledge of and work collaboratively with available community resources to provide a system of wrap around support for clients; engage new community referral sources and make community referrals as necessary
  • Copying, faxing, emailing, scanning, and filing physical and/or electronic records
  • Accurate and timely data entry and reporting in appropriate database(s)
  • Carry out assigned program(s) in compliance with grantor/funder requirements, including but not limited to, population served, budgetary and time restrictions, reporting requirements, etc.
  • Maintain organized, accurate, and current client files, including but not limited to, thorough progress notes, applications, forms, etc.
  • Attend meetings, supervision, trainings, and professional development opportunities as required
  • Provide departmental and agency-wide assistance and/or coverage during high volume programs or colleague absences
  • Comply with outlined policies, procedures and agency culture including representing TEEG positively at agency events and in all public venues
  • Identify and coordinate with available volunteers to support programming as needed
  • Accurate and timely logging of monetary and/or in-kind donations directly received

Responsibilities
  • Work collaboratively with Client Care Manager and other case management staff to provide individualized case management services to eligible clients as needed
  • Meet with current and referred clients to complete intakes, assessments, income verification, program qualifications, enrollments, ongoing case management, and case closings as needed
  • Enroll and maintain virtual accounts for available case management application services
  • Provide in-person, virtual, and/or telephonic supports to clients as needed
  • Become CHOICES certified within 6 months
  • Work with upper management to determine appropriate client relief allocation awards
  • Develop, maintain, and assess applicable program materials as needed
  • Provide outreach to the local community.
  • Create and distribute outreach and promotional materials for clients, community resources and public consumption to promote senior programming
  • Provide front desk support as needed
  • Perform administrative support or coverage for other program staff when needed
  • Any other duties as assigned by Supervisor or Executive Director
