Job description

Overview

The Telehealth Registered Nurse (RN) is a member of the Nursing Care Team and is responsible for collaboration, communication & facilitation of optimal provision of care through an audio-visual technology platform supporting daily management of care. Under the supervision of the Patient Care Services Nurse Manager, the Telehealth Admission Discharge RN is responsible for collecting clinical information and ensuring that all documentation is in order for admission, discharge and/or transfer of patients. The Telehealth Admission Discharge RN nurse will also provide direct patient care as necessary.

Position is 100% On-Site at the Hospital.

Responsibilities
  1. Demonstrates competency in obtaining an accurate patient physical assessment and medical history and documenting in EMR.
  2. Develops, discusses and communicates a realistic plan of care for each patient in collaboration with each patient and/or support person to address identified needs.
  3. Demonstrates the critical thinking skills necessary to implement a plan of care and nursing interventions as necessary for the safe care of the patient.
  4. Provides individualized educational opportunities and documents outcomes; educates on operations treatment plan and diagnosis when necessary.
  5. Prepares patient for discharge including required paperwork, discharge instructions, medication education and follow up appointments.
  6. Coordinating patient care with discharge planning team ensuring timely transition to next level of care.
  7. Collaborates with healthcare team.
  8. Demonstrates flexibility in managing hybrid position.
  9. Meets performance expectations for Customer Service, Teamwork, Resource Utilization, and Staff and Self Development as outlined in performance review.
  10. Performs other duties as assigned or directed to ensure smooth operation of the department/unit.
QualificationsEducation
  • Graduation from an accredited school of nursing; BSN preferred. Proficient in use of computer software to include electronic medical records.
Experience
  • Minimum 3 years of recent clinical practice experience in an acute care hospital.
  • Minimum 3 years of recent experience with the use of an electronic health record.
  • Strong computer skills. Orientation provided.
Licensure
  • Licensed and currently registered nurse in the State of Connecticut.
  • BLS certification required.
Additional Information

Virtual RN will be assigned to two units per day (max 60 patients) and will be responsible for completing discharges and admissions via video calls. Additional tasks such as audits.
