Position Summary
Acadian Health is seeking experienced Registered Nurses to provide hospital-level acute care in patients' homes as part of our innovative Hospital-at-Home program. In this role, you'll combine clinical expertise, technology, and collaboration with a virtual care team to deliver high-quality care outside of a traditional hospital setting. You'll serve as a critical link between patients, physicians, and interdisciplinary care teams while helping improve outcomes and reduce hospital stays.
Who Will Be Successful in This Role?
The ideal candidate is an experienced acute care nurse who enjoys working autonomously, collaborating with interdisciplinary teams, and providing high-quality care outside of a traditional hospital setting. Nurses with Med/Surg, Telemetry, Critical Care, Emergency Department, or Hospital-at-Home experience are especially encouraged to apply.
This position requires an active RN license, current BLS and ACLS certifications, and a minimum of 3 years of acute care nursing experience (Med/Surg, Telemetry, Critical Care, or Emergency Department). Candidates without recent bedside acute care experience will not be considered.

Pay: $42-45/hr.

Minimum Qualifications (Required)
Please apply only if you meet the following requirements:
- Active, unrestricted RN license in good standing
- Current BLS and ACLS certifications (American Heart Association)
- Minimum of 3 years of recent acute care nursing experience in:
- Medical-Surgical (Med/Surg), and/or
- Telemetry, Progressive Care, Critical Care, or Emergency Department settings
- Experience managing complex patient conditions independently
- Strong clinical assessment and decision-making skills
- Experience using electronic health records (EHR)
- Valid driver's license and reliable transportation
- Comfortable providing care in patients' homes and working independently in the field
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Emergency Department, Critical Care, EMS, Mobile Integrated Healthcare, or Hospital-at-Home experience
- Experience as a preceptor, educator, or mentor
- Experience with virtual care platforms or remote patient monitoring
- Community Paramedicine knowledge or certification
Key Responsibilities
In-Home Acute Patient Care
- Deliver hospital-level nursing care in patients' homes for conditions such as heart failure, pneumonia, infections, and post-surgical recovery
- Perform clinical assessments, treatments, medication administration, and patient monitoring
- Identify changes in condition and escalate concerns appropriately
Care Coordination
- Collaborate closely with physicians, advanced practice providers, case managers, and virtual care teams
- Serve as the in-home extension of the command center care team
- Communicate patient updates and clinical findings in real time
Patient & Family Education
- Educate patients and caregivers on disease management, medications, treatment plans, and recovery expectations
- Reinforce care plans and support patient engagement
- Help patients recognize when additional medical attention may be needed
Documentation & Compliance
- Accurately document assessments, treatments, and outcomes in the electronic health record
- Ensure compliance with all clinical protocols, quality standards, and regulatory requirements
- Maintain timely and accurate clinical documentation
Emergency & Rapid Response Support
- Respond to urgent patient care situations in the home setting
- Deliver appropriate interventions under established protocols and physician guidance
- Support patient safety and continuity of care
Program & Team Support
- Participate in quality improvement initiatives and program development
- Support onboarding and mentoring of new team members as needed
- Represent Acadian Health professionally with patients, families, and healthcare partners
