At the same time, the two resources we need most to address this problem - caregivers and primary care providers - are stretched thinner than ever.

Savoy aims to make aging better by restoring agency to seniors and integrating caregiving into the heart of healthcare to allow seniors to spend more quality days at home - and fewer days in the hospital. Today, this includes going into individual homes as well as partnering with senior living communities to coordinate and manage clinical gaps for these seniors, their caregivers, and the care ecosystem. Savoy deploys a team of clinical experts both in-person and virtually paired with proprietary technology to drive clinical outcomes - without disrupting the current care.

Position Overview:

Savoy Life is seeking a dedicated Registered Nurse (RN) for up to 20 hour per week during business hours, to provide hands-on clinical support for seniors in various setting including individual homes, assisted living, independent living, memory care and beyond to support direct patient care as well as extensive care coordination. The RN will work with residents, POAs, primary care providers, community groups, on-site senior living leadership to identify care gaps and implement chronic condition care plans to improve resident outcomes.

Key Responsibilities:

  • Conduct assessments, ongoing check-ins, and other in-person care for residents within senior living communities and other senior setting to identify and support care needs.
  • Assist Savoy's Advanced Practice Providers in developing and implementing chronic condition care plans in collaboration with the clinical team.
  • Coordinate care for residents through collaboration with rimary care providers, specialists, POAs/families, and other care services to ensure seamless patient care delivery. Note for senior living communities, this include community Health and Wellness Directors and other community staff.
  • Partner with an internal team including a local Resident Care Coordinator, Partner Success leader, and virtual Advanced Practice Provider.
  • Maintain accurate and detailed documentation in Savoy Life's Electronic Medical Record (EMR) and other technologies.
  • Triage after hours patient inbound phone calls to further support care.
  • Provide clinical expertise and act as a resource for on-site staff to support high-quality resident care.
  • Operate with flexibility and adaptability in a new and evolving role within a growing startup organization.

Qualifications:

  • Active Registered Nurse (RN) license in Washington.
  • Must possess a valid and current driver's license.
  • Strong background in geriatric/senior care, with a focus on care assessments and care coordination.
  • Experience in care planning for chronic conditions, including collaboration with interdisciplinary teams.
  • Excellent documentation and electronic medical record-keeping skills.
  • Ability to work autonomously, manage ambiguity, solve problems, and adapt to new processes as the role and organization evolve.
  • Experience in working in senior living facilities is highly desirable.

Skills & Competencies:

  • Strong clinical assessment and care planning skills.
  • Ability to work closely with both clinical and facility teams to coordinate care.
  • Proficiency in using EHR systems and other technologies for documentation and reporting.
  • Must possess a valid and current driver's license & be comfortable driving around to different senior home settings including senior living communities as well as individual homes and beyond
  • Excellent communication skills and the ability to work collaboratively with a diverse team.

Hourly Range: $52-$60/hour depending on experience.
