Nest & Care is seeking a dedicated and compassionate PRN Registered Nurse (RN) with Trach care experience to join our home care team. As a PRN RN, you will provide essential nursing care to our patients in the comfort of their homes. This flexible role offers you the opportunity to make a meaningful difference in the lives of our patients and their families.

Nest & Care is a nurse-owned and operated level III agency offering 24/7 quality home care services.
Responsibilities:

  • Meets new and prospective clients and performs an initial assessment of service needs - This could be done virtual and/or in-person, depending on the client's request.
  • Develops a Plan of Care for all new clients and updates as needed
  • Communicates with care managers, caregivers, office staff, clients, family members and other medical professionals about client's needs and concerns
  • Consults with the Clinical Director regarding the supervision of services
  • Provides training to the clients regarding their care, when requested
  • Assures proper maintenance of clinical records and documentation in compliance with State and Federal laws
  • Performs client evaluations every 30, 90 or 120 days as needed by the client's Plan of Care
  • Performs caregiver evaluations every 30, 90 or 120 days as needed by state requirements
  • Provides skills evaluations for new staff during the orientation process
  • Trains new staff and existing staff
  • Provides Clinical Oversight
Qualifications:

  • RN State License MD with Trach care experience is required
  • Current CPR and First Aid certification
  • TB vaccination and Hep B vaccination
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
  • Clear criminal background and driving record
  • Two professional references within the healthcare field
  • Prior home care experience is strongly preferred
  • Self-starter with the ability to work independently and manage time effectively
  • Strong interpersonal skills, including a willingness to teach and mentor caregivers
  • Comfort with technology; prior experience with electronic medical records preferred
  • Ability to drive and travel to cases as assigned
