• Conduct detailed psychosocial assessments to identify clients' mental health needs, strengths, and areas for growth.
• Provide high-quality, evidence-based therapeutic services tailored to individuals, families, or groups, and create customized treatment plans that respond to client needs.
• Collaborate with an interdisciplinary healthcare team, including physicians, nurses, and other specialists, to establish coordinated care plans that encompass all aspects of clients' well-being.
• Advocate actively for clients, assisting them in navigating healthcare and community resources, ensuring they receive necessary support and services during critical times.
• Maintain detailed and accurate documentation of client interactions and treatment outcomes in our electronic health records system, ensuring compliance with regulatory standards.

Your primary focus will be on clients in a hospice care environment, which requires sensitivity, patience, and a commitment to providing compassionate support. You will work with individuals and families grappling with terminal illnesses and significant life transitions. Your role will involve not only therapeutic interventions but also emotional support, crisis management, and grief counseling.

• A Master's degree in social work from an accredited institution, ensuring a strong foundation in clinical practices.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.
• A minimum of two years of post-licensure experience providing clinical social work services, with a preference for experience in hospice care or similar settings.
• Proficient in conducting diagnostic evaluations and developing comprehensive treatment plans that reflect evidence-based practices.

In recognition of your valuable contributions, we offer a competitive annual salary ranging from USD 115,000 to 130,000, commensurate with your experience and caseload.
