Job description

Public Health Nurse

Position Title: Public Health Nurse Pay Grade: P-2 FSLA: Non-Exempt Division: As assigned Union Status: Union Position Summary With limited direction and supervision plans, develops and provides professional nursing services within assigned program areas. Acts as a resource and consultant to community and government agencies in order to provide eligible clients access to care. Supervision Exercised Provides no direct supervision to Health Department staff. Supervision Received This position is under the direct supervision of the assigned Manager and Division Director.

Essential Functions:

  • Applies public health nursing principles and theories, and draws from other sciences to for a theoretical framework for decision-making in public health nursing practice.
  • Assesses individual and family needs in conjunction with a multidisciplinary team.
  • Develops individualized client/family care plans.
  • Implements individual and family care plans.
  • Evaluates the effectiveness of client and family interventions towards achieving established goals.
  • Performs clinical nursing procedures.
  • Provides health education to clients, families, and groups.
  • Acts as a referral/resource person in health and human services.

Job Requirements & Qualifications A Bachelor's Degree in nursing from a college program which includes a practicum experience in public/community health nursing with one year of public health experience preferred, AND •Certification of graduation from an accredited School of Nursing, •A current Illinois license as a Registered Nurse by the Department of Professional Regulation.

Essential Physical/Cognitive Abilities

  • The ability to sit, flex, stand, reach, and bend.
  • The physical strength to lift, carry, and transport materials/equipment equivalent to thirty (30) pounds.
  • Visual acuity, auditory ability, and clear written and verbal communication ability.
  • Manual dexterity and the ability for data input into various computer programs.
  • The ability to maintain professional behavior at all times.
  • Special Requirements
  • Must reside within St. Clair County, IL or be willing to relocate to St. Clair County within six (6) months from hire date.
  • The ability to work necessary hours to meet the position requirements including evenings and/or weekends as assigned.
  • May require provision of services at off-site locations.
  • A valid Driver's License.
  • A dependable automobile for use in position.
  • Auto insurance in effect with at least the minimum limits of liability as outlined and required by the state of Illinois.
  • Adherence to all local, state and federal regulations pertaining to OSHA guidelines.
  • Health Department employees: o Are required to have a TB test upon hire and separation of employment. o Must be current on CDC Healthcare Personnel Vaccine Recommendations, which include Hepatitis B, MMR (Measles, Mumps Rubella), Td/Tdap (Tetanus/Diphtheria/Pertussis), Varicella (chickenpox) and Meningococcal if over 65. Influenza and COVID vaccines are recommended and encouraged, but not required at this time and maybe subject to change. All employees must adhere to the St. Clair County Personnel Code.
  • Position contingent upon continued grant funding.
