Job description

Village Caregiving - Des Moines, IA is looking for a PRN Registered Nurse to join their home care team. The RN will visit clients' homes in and around Des Moines, IA to document medical history, conduct health assessments, and develop personalized care plans for caregivers. The nurse is also requested on occasion to train new caregivers. The schedule for the position is flexible. Pay for this position is $60/hour for up to 2 hours per client assessment.

Responsibilities

  • Visit patients in their homes for one-on-one visits

  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients 

  • Assess patients and their environment to develop a care plan in order for the caregiver to have a blueprint to know how to safely and competently take care of our patients  

  • Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life

  • Provide effective communication to patients/clients, their family members, team members, and other health care professionals

  • Fill out VA Nurse Assessment documents on a supplied tablet the same day as assessment.  Upload Assessment to CRM. 

  • Keep timely communication with the Executive Director on schedule for upcoming and current assessments. 

  • Notify the Executive Director at the end of each pay period list of assessments completed.

Qualifications

  • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing

  • Current state license as an RN

  • Compassionate, friendly demeanor 

  • Excellent communication skills

  • Current driver’s license and reliable transportation 

  • Ability to work a flexible schedule and travel locally
