What You'll Do
  • Complete skilled nursing assessments and home visits
  • Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
  • Develop and update individualized plans of care
  • Provide wound care, medication management, and disease education
  • Monitor patient conditions and report significant changes
  • Educate patients and caregivers
  • Coordinate care with physicians, therapists, and other team members
  • Complete accurate and timely clinical documentation
  • Help patients safely manage their health at home

Qualifications
  • Active Washington, DC Registered Nurse License Required
  • Graduate of an accredited nursing program
  • Current CPR/BLS Certification
  • Strong clinical assessment and communication skills
  • Dependable transportation to patient visits
  • Ability to work independently in patients' homes
  • Home Health and OASIS Experience Preferred
