Position Summary: Licensed Practical Nurse (LPN) PRN
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team on a PRN (as-needed) basis. The LPN provides high-quality nursing care to residents while working closely with Registered Nurses, physicians, and the interdisciplinary team to promote each resident's health, comfort, and well-being in our long-term care and rehabilitation facility.
Qualifications
  • Current Licensed Practical Nurse (LPN) license
  • Current BLS/CPR certification required
  • Strong clinical and assessment skills
  • Experience in long-term care or rehabilitation preferred, but not required
  • Excellent communication and interpersonal skills
  • Compassionate, dependable, and resident-focused
  • Ability to work effectively in a team environment

Key Responsibilities
  • Provide direct nursing care to residents in accordance with individualized care plans.
  • Administer medications and treatments as prescribed.
  • Monitor residents' conditions and promptly report changes to the RN or physician.
  • Accurately document assessments, treatments, and resident responses.
  • Assist with admissions, discharges, and physician orders.
  • Collaborate with the interdisciplinary team to provide exceptional resident care.
  • Supervise Certified Nursing Assistants (CNAs) and support quality resident care.
  • Maintain compliance with all state, federal, and facility policies and regulations.
  • Promote a safe, clean, and compassionate environment for residents and families.
