Job description

We are seeking an experienced and compassionate healthcare professional with a background in hospice, or field-based patient care to join our team as a Tele-video Care Coordinator.

This role blends hands-on in-home patient care with telehealth coordination. You will visit patients in their homes, support virtual provider visits, perform basic clinical procedures, and coordinate ongoing care to ensure patients receive high-quality, continuous support in the comfort of their home.

This position is ideal for individuals who are experienced in home-based care and are comfortable working independently in the field while supporting providers through telemedicine.

Key Responsibilities

  • Conduct approximately 7–8 televideo patient visits per day.
  • Build and maintain strong relationships with patients and caregivers through ongoing virtual engagement.
  • Review and reinforce provider-directed care plans.
  • Document all patient interactions accurately within the electronic medical record (EMR).
  • Monitor patient progress and identify opportunities for intervention.
  • Coordinate care among providers, specialists, caregivers, and community resources.
  • Assist patients with appointment scheduling, referrals, and follow-up care.
  • Support patients enrolled in:
    • Chronic Care Management (CCM)
    • Remote Patient Monitoring (RPM)
    • Transitional Care Management (TCM)
    • Behavioral Health Integration (BHI)
    • Advanced Primary Care Management (APCM)
    • Remote Therapeutic Monitoring (RTM)
  • Escalate clinical concerns to providers as appropriate.
  • Help reduce avoidable emergency room visits and hospital admissions through proactive patient engagement and care coordination.

Qualifications

Required

  • High school diploma or equivalent.
  • Experience in healthcare, care coordination, patient services, medical assisting, or a related field.
  • Experience using Electronic Medical Records (EMR).
  • Strong verbal and written communication skills.
  • Comfortable conducting virtual patient visits and building rapport remotely.
  • Excellent organizational, multitasking, and time-management skills.
  • Ability to work independently and collaboratively within a multidisciplinary care team.

Preferred

  • Certified Medical Assistant (CMA) or equivalent healthcare certification.
  • Experience in primary care, geriatrics, chronic disease management, or population health.
  • Experience with eClinicalWorks (eCW).
  • Knowledge of Medicare care management programs.
  • Bilingual language skills.
