DaVita is hiring a Patient Advocate to support home patients with end-stage renal disease.
General purpose of the job:
This position's key function is to meet with a potential home patient and discuss his/her experience with the specific home therapy.
Essential duties and responsibilities:
• Discuss experience with the home therapy.
• Be a positive role model for new patients interested in a home therapy.
• Be a role model to new patients by learning information about treatments for kidney disease and following his/her own treatment plan.
• Topics for discussion can include their schedule, lifestyle, benefits, challenges and impact on their quality of life.
• Patient Home Ambassador will not discuss or provide medical advice or insurance information to the potential patient.
• At the facility's request, Patient Home Ambassador will meet with the new patient or potential new patient at the home facility. During the meeting a DaVita teammate will be present to guide discussion. • Volunteer position.
Minimum Qualifications:
• On Modality for 6 months: The Patient Home Ambassador needs to be on the home therapy a minimum of 6 months at a DaVita clinic.
• Medically Stable: The Patient Home Ambassador needs to be deemed medically stable for this role. Criteria can include a stable care plan by the inter- disciplinary team.
• Willing to discuss experience: Identify patients that are excited and motivated to discuss their experience and insight into the therapy.
• Positive attitude: The role of the Patient Home Ambassador requires a positive attitude to ensure and facilitate the communication process.
• Good Communications skills: The Patient Home Ambassador is required to have effective communication skills and be able to articulate his/her experience to the potential patient.
• Scheduling Demands: The Patient Home Ambassador has the ability to meet potential patients at the designated DaVita facility.
