Help

GS-3 (Entry level):

  • Learn to appropriately utilize glucose monitoring machine to collect and document appropriate glucose readings for assigned veteran
  • Maintain a positive therapeutic relationship by displaying respect for human dignity with patient, families and/or significant others utilizing learned communication skills
  • Utilize observation skills for data collection, reporting and recording patient/resident observations
  • Learn to performs aspects of nursing care to meet patient's needs for the following under close supervision and with direction/guidance from a preceptor:
    • a. Personal Hygiene
    • b. Physical comfort
    • c. Activity, Exercise, Rest
    • d. Nutrition
    • e. Elimination
    • f. Prevention of Skin Breakdown
    • g. Rehabilitative Aspects of Care
  • Report patient observations to the RN and or other members of the treatment team
  • Learn to utilize equipment and supplies according to procedures

GS-4 (Intermediate/Developmental Level):

  • Utilize observation skills for data collection, reporting and recording
  • Maintain a therapeutic environment for patients according to established procedures and policies by reporting actual or potential accidents, fire hazards and faulty equipment
  • Competent with performing aspects of nursing care to meet patient'/resident needs for:
    • a. Personal Hygiene
    • b. Physical Comfort
    • c. Activity, Exercise. Rest
    • d. Nutrition
    • e. Elimination
    • f. Prevention of Skin Breakdown
    • g. Rehabilitative Aspects of Care
  • Perform and document prescribed medical treatment and interventions for patients, such as diabetic urine testing and blood testing, simple dressing changes
  • Monitor patients according to levels of observations i.e. (one to one suicide, continuous observation, q 15-minute checks, and hourly rounding
  • Monitor Veterans who may be at risk for harming themselves and or others according to established policies and procedures and appropriately refers issues to the charge nurse or other health care professional

GS-5 (Full Performance Level):

  • Utilize glucose monitoring machine to receive glucose readings for veterans refers deviation to appropriate health care staff
  • Make observations of subtle changes in resident's condition, i.e. behavior shifts, mood, skin coloring, appetite, etc. and takes action accordingly within the NA's assigned role
  • Document care given, observations and responses on appropriate tools or forms
  • Perform nursing procedures within scope of practice and according to policies and standard procedures to care for the veteran for such procedures but not limited to: enemas, catheterization, colostomy care, skin care simple dressing, application of braces and prosthetic devices, monitoring of
  • Recognize the residents' autonomy and promotes independence by means of respect, limit setting and individualized treatment
  • Utilize equipment to promote the veteran self-care, such as; lift equipment, Hydro sound Tubs, and Veteran/Resident Wandering System
  • Verbally provides patient information to the treatment team for inclusion in the veteran/residents' plan of care, participates in treatment team meetings for resident care planning
  • Enter information about the residents' condition in the chart, utilizing appropriate formats
  • Remove defective equipment, reports hazards, observes safety regulations, contributes to infection control, and utilizes proper aseptic technique in performance of duties
  • Manage distributed patient behavior and by appropriately intervenes and request assistant from other health care members
  • Appropriately initiates basic life support, takes actions appropriate action

Work Schedule : Various, may include 8 and 12 hour shifts, days, nights, weekends, and holidays. Schedule will ultimately be determined by supervisor and facility needs
