What You'll Do

Provide Patient & Family Support

  • Assess patients' psychosocial needs related to illness, environment, and support systems
  • Develop and implement individualized care plans focused on emotional, social, and practical needs
  • Provide counseling, guidance, and support to patients and caregivers
  • Assist patients and families with navigating resources, financial concerns, and care planning

Connect Patients to Resources

  • Coordinate referrals to community resources and support services
  • Serve as a liaison between patients, families, and community agencies
  • Build and maintain relationships with local organizations to support patient needs

Collaborate with the Care Team

  • Partner with nurses, physicians, and interdisciplinary team members to ensure holistic care
  • Participate in case conferences and contribute to comprehensive care planning
  • Support discharge planning to ensure safe and successful transitions

Document & Communicate

  • Maintain accurate, timely documentation in the electronic medical record (EMR)
  • Communicate patient needs and updates clearly with the care team and referring providers
  • Uphold professional standards and regulatory requirements

What You Bring

  • Master's degree in Social Work (MSW) from an accredited program
  • Active licensure in the state of practice (as required)
  • 1+ year of healthcare experience preferred (home health strongly preferred)
  • Current CPR certification
  • Valid driver's license, reliable transportation, and auto insurance
