Community-Based Mental Health Therapist


About the Position

We are seeking a dedicated and compassionate Community-Based Mental Health Therapist to join our mission-driven team. In this role, you will bring high-quality mental health care directly to the individuals and families who need it most-whether in their homes, schools, or other community settings.

You will help clients build coping skills, strengthen support systems, and navigate life's challenges with confidence and resilience. This is an opportunity to make a meaningful impact every day by promoting mental health equity and delivering culturally affirming, trauma-informed care.

As a Community-Based Therapist, you will provide direct clinical services and support to individuals and families in their homes, schools, and other community settings. Responsibilities include:


Clinical Services

  • Conduct intake procedures, including all required forms and releases.
  • Provide diagnostic assessments to determine eligibility and treatment needs.
  • Facilitate individual, family, and group therapy using culturally responsive, evidence-based approaches.
  • Deliver crisis intervention, safety planning, and risk assessments when needed.

Treatment Planning & Collaboration

  • Develop collaborative treatment goals with clients and families.
  • Identify client strengths, needs, and barriers to inform the Individual Service Plan (ISP).
  • Modify treatment plans as needed and communicate progress during treatment team meetings.
  • Coordinate care with community partners, schools, and other service providers.

Documentation & Compliance

  • Complete treatment plans, progress notes, and discharge summaries in a timely manner.
  • Maintain productivity expectations and meet all state, federal, and funding requirements.
  • Participate in clinical supervision and ongoing professional development.

Community Engagement

  • Connect clients to community resources and ensure follow-up on referrals.
  • Support clients navigating trauma, addiction, grief/loss, family conflict, chronic stress, and other challenges.
  • Provide services where clients feel most comfortable-home, community, school, or telehealth.


Qualifications

* Bachelor's degree in Social Work, Psychology, Counseling, or a related mental health field.

  • Ohio licensure required: LSW, LPC, MFT (independent licensure preferred-LISW, LPCC, IMFT).
  • Passion for serving underserved populations and addressing mental health disparities.
  • Strong engagement skills, cultural humility, and the ability to meet clients where they are.
  • Commitment to ethical, person-centered, trauma-informed care.
